The forms for the Family-Player Tribute Ads for the
2017 Game Program are included below.
The deadline for submitting a request is
Wednesday August 16.
1) Please fill out Page two of "Family Player Tribute Ad" doc below
2) Send it in with Payment.
3) Select the desired page size, (FULL, HALF, or Quarter)
4) Use the appropriate "Ad Document Format" to build your page
5) Senior dedication forms are also due on August 16
6) ONE LINERS are due on Tuesday August 22nd. See the attached form ($1 each due)
7) E-mail the forms back to phhsfpa@gmail.com.
8) Please forward this info to any friends and relatives

Arrive at 4:30p for concession stand food prep/setup and field operations decorations. (Cups, Flags, banners, etc..) Please help to make this HOME game vs Jefferson a HUGE EVENT!
THE FAN ZONE
Will be open for Vikings Apparel and other items (game programs, Merchant cards, etc)
Cash or check is preferred.

Reminder: All event parking is in the main front lot of PHHS. Tennis courts and Lots behind the school are reserved for Officials, School staff and Handicap.
